Trivia question: Does USCIS appoint MD's and OD's to be Civil Surgeons without even trying to make sure they care enough to at least read and understand instructions? Now question for real, about A-number:My husband's Alien Registration Number can be found on his old EAD but he is not sure if it is still OK to use if EAD has expired.Is it? I never had EAD.My VISA was last stamped 7 years ago and they did not put A# on VISA's than. Should I use I-94 number instead or nothing at all?[/quote] This doctor seems to be VERY incompetent if she cannot properly fill out a simple form. I am sure you must be wondering how she passed her MD exams. I am glad you went to another doctor who seems very knowledgeable. Ask your new doctor to provide you a copy of the I-693 form which was sealed earlier. Tell him that a copy of that was not given to you. You should report this doctor to USCIS. Not sure if that would help; but every feedback would be helpful. BTW, you are required to provide sealed copy of I693 only to USCIS. It would be fine if it is open when you give it to new doctor (though let it remain sealed if you are meeting him tomorrow). For A# - You can actually find it on your I-140 approval notice. Yes, he can use it from his old EAD card. They should be the same as I-140 approval notice. A# uniquely identifies applicant in the USCIS systems.

[/quote] Yes you can choose any doctor you want. You do not have to stick with the previous doctor. I would recommend selecting a different doctor, if possible. There is no point in paying them money so that they can yell at you. As I suggested to Svitlana, you should also report this behavior to USCIS. You mentioned you have to go to collect results in few days. I am guessing that means they have already done all the tests. In that case you only have to collect the test results. You havent mentioned if you have already paid them. Typically they charge you at the end (when you collect the result). He should not give any bad report because he became upset. He should only honestly report the test result. He is also required to give YOU a FULL copy of I-693 form/report before he seals it. So you can easily find out what he wrote.

[biggrin][/quote] No one showed up in the end to tell us we were punked [biggrin] other than that I am am not sure of anything about that place.We were lucky to witness our needles for blood test came from a sealed package. Now, when I think back about how everything there looked, I start having this feeling that it is some "Dr. Feelgood" type of operation. Interior of the place looked very nice, like they make a ton of money.The field is Occupational Therapy and other than a MD capable of writing prescriptions staff consists of gorgeous young nurses or receptionists and a few burly guys. They had a lot of people in a waiting room,mostly jolly older man and a few young dudes that looked totally out of it. Well,you know hindsight is 20/20 and I also have a very active imagination as you can tell ))) Thank you Amazon, I found my A# on I-140 approval notice.

Attachments have been enabled on all forum posts. Here are the steps: 1. Click on "Post Reply" (quickly reply would not work) 2. Create a new post and check box called "attach files to this post" 3. Click on post button 4. It will ask you to select files to upload. 5. Select all images and click on upload. I am eagerly waiting for hilarious I693 form [laugh]

Svitlana & amazon, I may have to do what your doing and go for second exam. I was told today by his nurse when i called they don't give out copies of the medical exams and it will be sealed when I pick it up. However, she will let me know if anything was wrong. Ill see how things go tomorrow or just go to another doctor

Img006.jpg: She wrote that TB test results was negative on Aug 16. In addition she wrote that X ray was performed and data read on Aug 15. She also wrote that "an applicant has no physical signs of TB". Funny things: 1. She claims X ray was done even though it was negative. 2. X ray was done one day before the skin results. 3. In comments, she did not say which applicant (just an applicant)? 4. Img007: She added some random guys image on back of your form? It looks like she is reusing old papers and printed your form on that big photo. What did the new doctor say about this? Did he make any comments? Aren't you doubly glad you did not send that I-693 form directly to USCIS? [biggrin]

On page 4, it clearly states that they have to keep one copy and give second copy to applicant. http://www.uscis.gov/files/form/i-693instr.pdf I would recommend give this to them (early tomorrow morning) before they seal the I693 form.

She tested us for both AIDS and syphilis and you are not supposed to be tested for AIDS any more since 2009. She missed the fact we were not up to date with our MMR and Tetanus shots. So I got poked with four needles today:two for vaccine-one in a shoulder one in a butt,one for TST and one to draw blood again. Just go and do new tests.Forget about old doctor and wasted money and think about how you will get your GC soon))) But damn I could use my wasted $300.[lol]

[biggrin] [/quote] New doctor did not say anything at all.He just listened and than explained to us how he will make sure our I-693 is done according to instructions.He did not want to look at her papers. I wonder if there is some rule and if he did see violations of laws or doctors ethics code he would have to report it. He saw our cheap yellow envelopes with scribbles on them,sealed with wrinkly thin scotch tape that already started to peel off,and showed us custom made and printed envelopes his office uses for immigration exam. He did not tell us to trust him,or to not get freaked out if our papers got returned to us because it is easy to fix.Other doctor did. This one did not have to. For all of you guys who reads this.You know that feeling when you start debating in your had and convincing yourself :maybe it is not as bad as I think,maybe it will be OK,maybe I am just overreacting? Do not fool yourself, you know it is not OK and you just being a sucker. I knew from my first phone call to make an appointment there were things that made me cringe about that place, but I did not want to wait and other doctors did not have any appointments last week. So here I am a sucker.[lol]
